
Libby Carpenter- Patient Family Advocate
Since Libby graduated with her degree in Social Work and Mental Health in 2004, she has been serving the adult and senior community by providing assessments, case management and client coordination services.

Her work history includes: Director of Social Work in Premier skilled nursing centers, Vice President of Right At Home In Home Care & Assistance, Lily Hospice serving as a hospice advocate and currently owns and operates her own Elder Care Consulting Business- Guiding Seniors Advocacy and Education, LLC
​
She currently holds a board position with the Senior Coordination Aging Network (SCAN Oakland) (2010-present) and is a Certified Dementia Trainer through Second Wind Dreams since 2011.

Libby continues to have a strong commitment to the community and enjoys educating healthcare professionals on issues that affect our industry as well as helping the general population to understand resources available to them in the tri-county area.
She is a patient advocate both personally and professionally. Guiding Seniors Advocacy and Education, LLC was conceived to assist clients and families in navigating the fine art of aging and health care system.
Assisting clients in listening to their individual needs and wishes and formulating a individualized care plans as well as resources to meet the client where they are and assure preparedness for what may come.
In my spare time, I enjoy my family, friends, traveling, gardening and volunteering.
